Second Home is a multi-cultural Adult Day Health Care (ADHC) center located in Watertown, MA. Our goal is to provide compassionate, culturally-sensitive care and services to help our members remain healthy, independent, and safe in the community. Our services will ensure and improve their quality-of-life while reducing their need for frequent hospitalizations and long-term placement. Because we are a multi-cultural center, we have employees who are able to speak Russian, Armenian, and Farsi with participants.
We are committed to excellence and superior service delivery. Our experienced team has a proven track record of delivering personalized, high-quality healthcare and social services to our participants. Our interdisciplinary team addresses not only our members' medical needs, they also design personalized therapeutic activities to meet their specific needs and interests. In addition, our team focuses on improving our members' strength, improve memory and cognition, and most importantly their independence.
We accept MassHealth Standard, United Health Care SCO, Fallon Health SCO, Tufts SCO, Senior Whole Health SCO, and other approved SCO's.
We provide complimentary transportation to and from the residence. We provide nutritious meals: breakfast, snack, and hot lunch. All meals are created by the registered dietitian and approved by the Department of Education. We have trained Registered Nurses on staff at all times providing a safe/secure environment for our members. Our Nurses assess, monitor all vitals, assist, supervise, and educate with all ADL's.
We offer recreational and therapeutic activities are tailored to your individual needs and designed to help improve and maintain your level of independence and wellness. We offer activities that are cognitively stimulating, help enhance your memory promote your physical fitness, and create opportunities for your unique self-expression.
We provide case management, advocacy to help you obtain services from other community agencies, and interpreter services when you receive mail or other correspondence from health care and government agencies. Our social workers coordinate with our nursing staff, therapeutic activities staff, and alternative therapists to help you attain or maintain your best possible physical, behavioral, and psychosocial well-being.
